NOT SURE how it happened. he was shooting and the choke came lose so he reached up to tighten it and the gun went off got his thumb/pointer & middle finger. A wrench wouldn't have prevented that...Anytime I have to look at/work around the business end of a gun, I make DAMN sure it's unloaded. Physically take the barrel off the stock/action is what I do. It's more work, but it's worth it.
