I have had this problem a couple of times recently. I know its been wet recently, but is there anything to do to avoid this?

I've never heard of one getting mold and mildew in it.

How do you prevent it?

Use it more! Get it HOT to sanitize it (600+) and kill the spores, then use it a time or two every week.

Mine have such a coating of grease and soot on them that I doubt anything could grow on the ceramic, but I also use them regularly.

I've never had a problem like that with mine. Whether covered or not. I kept it covered for about a year until the cover cracked. Now it's been sitting uncovered for several years.

Mine does it too. Leave the bottom vent open enough to let some fresh air flow thru.

Yep it's covered and yes it gets rained on. Going to leave the top and bottom open...and use it more. Grillin isn't as much fun when it's 30! Grillin tonight because its 70.
