It seems that every picture on here get's the term "photoshopped" thrown at it.
Like others said, it's just another deer further away, in the background. No photoshopping needed.

The little deer in the PIC is just much further away from the camera than the larger looking doe. It's just in the right spot to make it look like it's in the does mouth as it walks toward the camera. This ^^^^^^^^^^
