Ok, my original thought was 4.5.

The last cusp on last tooth is beginning to slope to outside of jaw, the lingual crests of tooth 4 are pretty rounded, the dentine, looks to be 2 times as wide as the enamel on tooth 4

First two premolars have more wear than a three typically has.

The third premolar is replaced after 18 months old and it has more than 1 year of wear.

The last cusp on the last tooth is cupped too much to be a three.

And then the fact that the first molar has a lot of enamel showing, followed by the second molar having an equal amount of enamel to dentine. That in and of itself was enough but you add the three reasons above it to the mix and he’s 4 all day.
