I saw this on another website, he claimed that the string slipped off the cam or wheel. That hurt!

I find the "string slipped" explanation a bit questionable.
Even if it did, how would his hand have gotten in front of the arrow to penetrate all the way to the fletching?

Looks to me as if he is a lefty...and was probably supporting the arrow with his hand instead of the arrow rest....

Hmmm. Initially - Looks like a Right hand and the arrow is in the part of the hand that would be behind the riser when shooting left handed....
