If someone is talking on the phone with it up to their ear somewhere public, it doesn't really bother me unless I am trying to interact with that person specifically (cashier, etc). But, the talking on an ear piece that is not real visible just annoys me because it is so commonplace that if someone is actually trying to talk to me for some reason I just ignore it because I, too, have thought too many times that someone with an ear piece in was trying to talk to me. The whole speaker phone drives me nuts! There are some that swear they hear better over the speaker on the phone than with it up to their ear. That may be true for those people, but for sanity's sake, don't do it in public. The other problem with the talking over your speaker on the phone is that the person who is calling you may not want the whole world to hear the conversation.
That being said, I will talk over bluetooth in my car when I am driving, but I am usually by myself and if not, the calls are generally brief and whoever the conversation is with knows that they are being heard by more than just myself. And, if I call someone and ask what they are doing and they say they are driving somewhere, I just assume I can be heard by anyone else in the vehicle.
