That would ABSOLUTELY happen at my neighbors house - my wife and I are always amazed that their deck is still standing. We keep waiting for the fire department and ambulance to arrive and try to save them from a 10' fall as their deck collapses.

I promise you that deck was nailed to the home. Or used screws. There is a reason properly sized lags/carriage bolts with washers are required. There's a reason that no AHJ that I know of has approved the new ledger screws.

And that right there is the reason. I beat decks to pieces in my reports. I grade against DCA 6 standard, which is considered perfect. I find maybe 2 perfect decks a year and none so far this year. We do over 1000 inspections a year as a team.

Wouldn’t posts be a better option than a ledger board ? See a lot of decks fail with ledger boards nailed to the side of the house.

It's not the board that's the problem. It's the way its attached to the house which causes the problem.
