could be the mounts binding internals from tightness or torque,or some other alignment issues other than just a bad scope. have to check it all. If receiver is drilled off ,compensation can be had with Burris signature rings using offset inserts.
How good is your gunsmith? Were the rings lapped,aligned,and torqued with a torque wrench,or were they just installed?

Way too many possibilities of things that could be wrong.
Without being able to look at it no one here can say with any certainty what may be wrong.
You're best bet would be to take it back to the 'smith and let him figure it out... Assuming he knows what he's doing to start with.

To understand what you are saying, with all the adjustment in the scope, you maxed out the elevation and you are still 3 inches low? Do you have any MOA built into the base ? There should be plenty of elevation to get you zeroed, I will be interested to see if new rings make a difference.

Put a multi slot Weaver base and some Burris Signature Rings on it. It's just a better system with more adjustment all the way around and in every way.
Unless you had to move the scope way over to the left/right as well... it is highly unlikely that the barrel is crooked.
Either the scope of the mounts. Likely the mounts.
No government employees were harmed in the making of this mess.

You didnt by chance by the 20MOA rings did you? If you did then the smith put them on in the wrong orientation. That's kinda what I was thinking too. Either that or like R H said, the rings are mixed up. I would hope whoever mounted it would catch either though. I doubt the barrel is crooked

trying out another optic is always a good way to find out if it is the scope or mount too? That scope has plenty of vertical to get you where you need to be, I'm guess something in the mount. I do look forward to hearing your resolution. The Meopta's my friends and I have messed with have all been very nice scopes. Well built and very nice glass. I just ordered one with the new Dichrotic reticle, it should be excellent for hunting.

It's odd that it required any shim.
I've mounted quite few scopes on Vanguards using Talley rings and had no problems
